TRICARE East Referrals and Pre-Authorizations
How to Get a Referral
A referral is required when your primary care manager (PCM) sends you to another provider for care they do not provide. A pre-authorization is when Humana Military approves your care before your appointment.
Your PCM will initiate the referral and pre-authorization at the same time. Once approved, Humana Military sends you an authorization letter with specific instructions. You can find this letter in your patient portal.
Key rules for referrals:
You must book your appointment with the provider listed in the authorization letter.
If you need to find another provider, contact Humana Military.
You must get care before the authorization expires. Otherwise, you need to get it reapproved.
Active duty service members need a referral for most care received outside their assigned military hospital or clinic.
How to Check Referral Status by Phone
Call 800-444-5445 to check the status of a referral or authorization. You can also sign in to your Humana Military secure portal online.

Not having your DOD Benefits Number ready. Your DOD Benefits Number (found on your ID card) or Social Security number speeds up verification.
Submitting claims to the wrong address. East Region claims go to P.O. Box 202146, Florence, SC 29502-2146. West Region claims go to a different address.
Expecting 24/7 phone service. The main Humana Military line is available 8 a.m. to 6 p.m. ET/CT, Monday through Friday. The Nurse Advice Line and the Veterans Crisis Line are the 24/7 options.
Forgetting to check the patient portal. The Humana Military portal often provides faster answers than the phone line for referral status and claims.
